Read Carefully before Using
The Public Health Service Act (Section 308 (d)) provides that the data
collected by the National Center for Health Statistics (NCHS), Centers
for Disease Control and Prevention (CDC), may be used only for the
purpose of health statistical reporting and analysis.
Any effort to determine the identity of any reported case is prohibited
by this law.
NCHS does all it can to assure that the identity of data subjects cannot
be disclosed. All direct identifiers, as well as any characteristics
that might lead to identification, are omitted from the data set. Any
intentional identification or disclosure of a person or establishment
violates the assurances of confidentiality given to the providers of the
information. Therefore, users will:
1. Use the data in this data set for statistical reporting and
analysis only.
2. Make no use of the identity of any person or establishment
discovered inadvertently and advise the Director, NCHS, of any
such discovery.
3. Not link this data set with individually identifiable data from
other NCHS or non-NCHS data sets.
By using these data, you signify your agreement to comply with the
above-stated statutorily based requirements.

Introduction
The 1993 National Mortality Followback Survey (NMFS) is the sixth mortality
followback survey conducted by NCHS. The multi-component survey focuses on a
national representative sample of individuals aged 15 years and over who died
in 1993. Forty-nine of the 50 State vital registration areas granted approval
to sample their death certificates. The independent vital registration areas
of the District of Columbia and New York City also gave permission to sample
their death certificates. South Dakota declined to participate in the NMFS
due to state law restricting the use of death certificate information. Data
for the 1993 NMFS are therefore representative of individuals aged 15 years or
older who were residing and died in the United States in 1993, excluding South
Dakota.
The files listed below comprise the 1993 National Mortality Followback Survey (NMFS)
provisional data release. The provisional data files are in ASCII format. The documentation
files are in WordPerfect 6.1 format and the survey instruments are in .pdf format. We also
included the following files in ASCII format: a SAS input file ; a variable list with labels; sample
SAS and SUDAAN programs; a list of anaylsts and their research interests; a list of publication
from previous NMFSs;ICD-9 codes; and the Adobe Acrobat Reader 3.0 software.

Overview of the File Structure
The 1993 National Mortality Followback Survey (NMFS) public use provisional
data file contains 22,957 records with a length of 2,623. Each record contains
data for a single decedent. The contents of the file are described in detail
in the file documentation which also contains question text, and weighted and
unweighted frequencies for each variable. Each record is divided into four
parts. These parts contain control and sampling information, death
certificate data, proxy respondent questionnaire data, and multiple cause of
death data. Each part is further sub-divided into groups of related
variables.
