Remarks:  This is the file that is used to standardize the charges in building the Relative Weights.  
It is created at the same time the Provider Specific File is and is used in the AOR/BOR runs.  
A new file is created for each ,the NPRM  and for the Final Rules. 
The records are in provider number sequence, it is possible to have missing values in some records.
