		4 = 	Independent laboratories

		5 = 	Clinics (multiple specialties)

		6 = 	Groups (single specialty)

		7 = 	Other entities

		NOTE:   PRIOR TO VERSION H, DME claims also used this code; 
			   the following were valid code values:

		0 = 	Clinics, groups, associations, partnerships,
